Family-Managed Home Care program from the HCCSS

To allow families to have greater flexibility and choice of home care provider, the Home and Community Care Support Services (HCCSS) (previously LHIN and CCAC) has launched the Family-Managed Home Care program whereby families receive money directly from the HCCSS each month to pay for home care nursing or personal support services.

Families can hire Guardian Home Care for the Family-Managed Home Care program. We can deliver the highest quality Certified Personal Support Workers (PSWs) and/or Registered Practical Nurses (RPNs) for home care, and the HCCSS will give you money to help pay your invoice.

What is Family-Managed Home Care?

Family-Managed Home Care is a new model for home care delivery in Ontario that is available in each HCCSS. It is a self-directed care program. Family-Managed Home Care is intended to provide more choice and flexibility over home care to eligible clients and families. Through the program, eligible clients, or their substitute decision-makers, will receive funding each month that they can use to purchase home care services.

Clients or substitute decision-makers are responsible for managing and coordinating their care providers and managing funding as well as reporting to the HCCSS. Clients continue to be assessed/re-assessed by the HCCSS Care Coordinators and the HCCSS remains responsible for developing the client’s care plan/plan of service and monitoring outcomes.

Who is eligible for Family-Managed Home Care?

Clients must first have an assessment by a HCCSS Care Coordinator to determine their needs and ensure they meet eligibility requirements for traditional home care. The HCCSS Care Coordinator will develop a care plan/plan of service that would then be followed by Guardian Home Care PSWs and nurses.

To be deemed eligible for Family-Managed Home Care by the HCCSS Care Coordinator, clients must belong to one of the four groups identified by the Ministry:

  1. Clients in extraordinary circumstances – as assessed by the LHIN care coordinator. Meaning seniors needing personal support and nursing help at home;
  2. Children with complex medical needs;
  3. Adults with acquired brain injuries (ABI); and
  4. Eligible home-schooled children.

How do I apply for Family-Managed Home Care?

Intake begins through the HCCSS general intake phone number at 416-506-9888. They will ask you a series of questions to determine your eligibility and create a file for your loved one. An in-person assessment with a Care Coordinator will be scheduled and a care plan developed by the Care Coordinator. Guardian Home Care can then be hired to carry out the care plan and any additional needs of the client.

How many hours of service can be funded by the Family-Managed Home Care program?

The number of hours per week/month that are paid for by the HCCSS will be determined by their Care Coordinator during the assessment. A Care Coordinator can grant up to 21 hours per week, with more hours per week being granted with approval from higher management at the HCCSS. Nursing home care services can also be covered by the HCCSS with this program.

Our minimum shift length for Personal Support Services is 4 hours. However, we can provide any length of care you need up to 24/7. Nursing Visits are not subject to a minimum shift length.
