Our approach

Care built around stroke recovery

Every stroke is different. Our care plans start with a Registered Nurse assessment and adjust as your loved one regains strength and ability at home.

Nurse-led care plans

A Registered Nurse builds and supervises every care plan, coordinating with your family and health team throughout the recovery.

Daily rehabilitation support

Our caregivers encourage prescribed exercises and rehabilitation routines, keeping progress consistent between therapy appointments.

Family caregiver relief

We give family caregivers real respite, taking over personal care and supervision so they can rest and avoid burnout.

Frequently asked questions about stroke care at home

How do I care for a stroke patient at home in Toronto?

Start with a free nursing assessment so a Registered Nurse can evaluate the survivor's specific deficits and build a care plan. A trained caregiver can handle personal care, medication reminders, mobility support, and daily rehabilitation exercises, giving family members real relief. Our guides on stroke care at home in Toronto cover the essentials in detail.

Can stroke recovery happen at home?

Yes, for survivors who are medically stable enough for home discharge. Many stroke survivors make significant gains at home, particularly when care is consistent and exercises from the rehabilitation team are continued daily. Our caregivers support the therapy plan your physiotherapist and occupational therapist have built, maintaining the routines that protect and extend hospital gains.

What does a stroke care plan include?

A Guardian stroke care plan is built by a Registered Nurse and tailored to the individual. It typically includes personal care, mobility support, medication reminders, daily rehabilitation exercise encouragement, companionship, and monitoring for mood changes or warning signs of a second stroke.

How quickly can stroke care at home start?

We can usually arrange a nursing assessment quickly, sometimes the same day depending on the circumstances, and care can start soon after. If you need urgent support following discharge from hospital, call us directly at 416-613-8570.

Does HCCSS cover stroke care at home?

Ontario Health atHome (formerly HCCSS) may fund some hours of home care after a stroke. Guardian can supplement that coverage with private hours when public funding does not fully meet the survivor's needs. We help families understand both options.

A caregiver’s guide

Caring for a stroke patient at home

If you are caring for a stroke patient at home, you may have questions about whether your loved one will recover and what care will look like in the months and years ahead. Asking for help is one of the most important things you can do.

Coming home after a stroke

A few issues are worth addressing before the day of discharge.

Consider safety and access

Book a free in-home assessment from a Registered Nurse to identify what the home needs before your loved one arrives. You may need to relocate the bedroom, install a stairlift, remove rugs to prevent falls, or add grab bars and a shower seat. Guardian Home Care can help arrange it all.

Get ready for behaviour or mood changes

The losses from stroke, whether temporary or permanent, can be devastating to the survivor. Stroke survivors are at risk for depression. If you see signs of depression, ask the doctor for help.

The loss of life as it was before the stroke, and the rehabilitation ahead, can be overwhelming for everyone involved. A social worker from the hospital or rehab facility can support the whole family through this transition. If you are bringing in hired help, let them know how everyone is feeling and build some companionship and redirection into the stroke care plan.

Be aware of the risk factors for a second stroke

When caring for a stroke patient at home, help your loved one take medications as prescribed and keep all doctor appointments. A hired caregiver can take on this coordination, reducing the enforcement burden on family members.

Day programs in Toronto specific to stroke survivors

The Toronto Central Neighbourhood House hosts a Stroke Survivors Club at 349 Ontario Street. It is a social, recreational, and respite program for stroke survivors and their caregivers, focused on possibility and intellectual stimulation rather than loss.

The March of Dimes Afterstroke program connects you with a coordinator who can support your stroke recovery at home. Reach the Afterstroke Warmline at 1-888-540-6666.

Programs and hours change; call ahead to confirm current details before attending.

Assess the patient’s needs as well as your ability to meet them

The stroke survivor may need the following:

  • Personal care such as bathing and dressing, depending on the type of stroke experienced
  • Coordination of health care needs, including medications and doctor and rehab appointments
  • Supervision. Is your loved one able to stay home alone, even for short periods?

Try to be realistic about what you can sustain over the long term. A Personal Support Worker can handle bathing, grooming, dressing, and medication reminders so that the caregiving situation remains workable for everyone.

Connect with support groups

Take advantage of every opportunity to learn about stroke and your loved one’s prognosis. Talk with the health care team at the hospital or rehab facility about what stroke recovery at home looks like and ask for written information you can reference later.

The Canadian Heart and Stroke Foundation runs peer support groups for survivors and a separate group for caregivers of stroke survivors. Connecting with people in your situation is often the most practical source of information and encouragement.

A stroke care plan essential: taking care of yourself

The more you care for yourself, the better you can care for your loved one. Taking time for your own health is not selfish; it is what makes sustained caregiving possible.

You have a right to your own time and activities. Respite care gives you space to recharge. Allow yourself to feel frustrated or sad, and share those feelings with someone other than your loved one. Caregivers are also at risk for depression; if you think you may be depressed, talk with your doctor, as it responds well to treatment.

Many caregivers wait for permission from others before accepting help. Other people do not see the full weight of what you carry. You do not need permission to take care of yourself.
