Family-Managed Home Care Program
To allow families to have greater flexibility and choice of home care provider, Ontario Health atHome offers the Family-Managed Home Care program where families receive money directly each month to pay for home care nursing or personal support services.
Families can hire Guardian Home Care for the Family-Managed Home Care program. We can deliver the highest quality Certified Personal Support Workers (PSWs) and/or Registered Practical Nurses (RPNs) for home care, and the funds will assist in paying for the appropriate services under the care plan of the client.
What Is Family-Managed Home Care?
Family-Managed Home Care is a self-directed model for home care delivery in Ontario. It is intended to provide more choice and flexibility over home care to eligible clients and families.
Through the program, eligible care recipients, or their substitute decision-makers, receive funding each month that they can use to purchase approved home care services.
Care recipients or substitute decision-makers are responsible for managing and coordinating their care providers, managing the funding and reporting to Ontario Health atHome. Care recipients continue to be assessed or reassessed by Care Coordinators, who remain responsible for developing the client’s care plan and monitoring outcomes.
Who Is Eligible
Clients must first have an assessment by an Ontario Health atHome Care Coordinator to determine their needs and ensure they meet the eligibility requirements for home care.
Eligible groups may include:
- Clients in extraordinary circumstances
- Children with complex medical needs
- Adults with acquired brain injuries
- Eligible home-schooled children
Eligibility and funding are decided by Ontario Health atHome based on the unique needs of the care recipient.
How Do Families Use the Funding?
Once the funding is approved, the family may use it to pay for eligible personal support or nursing services. It is their job to hire care providers, arrange scheduling, pay for the services, and complete all the necessary financial reporting.
The number of hours and services funded will be determined by the Care Coordinator during the assessment. It is essential for families to confirm what services and payments are covered before arranging care.
